School Notes

This form is to be completed by the parent of caregiver of the student/s for leave of more than 10 days for the purpose of travel within Australia and/or overseas.

If your child has a medical condition that the school needs to be aware of, please complete this form and return it to the office.

Please complete the relevant forms if your child requires medication at school.

Please ensure your child complies with the School's Uniform Policy.

Parents can complete this form and return it to school when their child has been absent from school. (If the absence is for a holiday from 10 to 100 days, then families are required to submit an application for extended leave.)

Information regarding students riding their bikes or scooters to school.

When travelling between home and school, students must adhere to various regulations regarding passenger behaviour and the codes of conduct for travel by bus.

If you would like to meet with your child's teacher, or discuss a matter with them over the phone, please fill in this form and submit it to the office. The teacher will be in contact with you in du course.
